How to Verify Coverage

Before your first appointment, I recommend that you:

1. Call your insurance provider or check your online member portal.

2. Ask if Medical Nutrition Therapy (MNT) is a covered benefit under your plan.

3. Confirm that Refined Wellness Nutrition is listed as an in-network provider.

Please note: It is the client’s responsibility to verify if Medical Nutrition Therapy services are covered by their insurance plan. If services are not covered, the client will be responsible for the cash pay rate.

Out-of-Pocket & Superbills

If I am not in network with your insurance you can still be seen as a cash pay client. I can provide a superbill you may submit to your insurance for possible reimbursement.

Cash Pay Rates

Payment is due in full at time of appointment

• Initial Appointment: $175

• Follow-up: $130

Cancellation Policy

A $50 fee with apply for:

• No-shows

• Cancellations made within 24 hours of your scheduled appointment
